Acute Bilateral Vestibulopathy with Concomitant Progressive Deterioration of Binaural Hearing

Bilateral vestibulopathy is a condition with vestibular hypofunction of both inner ears. Patients this diagnosis will often complain dizziness and/or imbalance in darkness and when walking uneven terrain also experience oscillopsia. Predominant etiology idiopathic. A 73-year old man complaints for 2 days. Objective findings included spontaneous nystagmus, positive Romberg test eyes closed, pathological video head impulse test. Initial audiometry only revealed bilateral presbycusis. Following gradual non-complete remission vertiginous symptoms, the patient was discharged scheduled follow-up. The later readmitted due to progressive hearing deterioration alongside persisting symptoms. Various additional tests all came out negative, classified as idiopathic acute concomitant binaural hearing. referred cochlear implantation. Acute monosymptomatic difficult diagnose, it requires very specific that are not routinely done by neurologists. leading anacusis indeed so rare has been possible find any literature describing similar case.
